    The Dutch Safety Board is authorised to conduct safety investigations in nearly every area and sector. The Board is independent and aims to contribute to learning from incidents, accidents and disasters. In the essay, the authors reflect on what it means to conduct independent investigation in the public interest. They define independent investigation as a relational enterprise that requires reflexive investigators but also elementary and advanced safeguards in position, judgement and presentation.

    Debates about the quality of local democracy are not new. This was also the case 100 years ago. In 1918 the ground-breaking dissertation of Adriaan Buriks on democratic municipal government was published in the Netherlands. In his dissertation Buriks proposed a radical democratic reform of Dutch municipal government by adopting the new system of ‘commission government’ from the US. That is a system of city government in which (almost) all city authority functions are concentrated in a small commission. The commission is organized in such a way that each member of the commission is the head of a city government department, every member is chosen by the population of the whole city and the exercise of authority is controlled by the whole population. The essay describes the remarkable life of this social revolutionary and municipal law expert. It goes on to discusses the dissertation through an introduction on the central issue of democratic municipal government, a description of the government systems of the US before and after the American Revolution of 1787, a description of the new system of ‘commission government’ and his proposal to adopt the new system in the Netherlands. The essay is closed by the author’s reflection on how the dissertation was received and its impact.

    During a disaster or crisis, the response capacity of the government is put under extreme pressure. At the same time, citizens are often resilient in times of crisis and are increasingly capable of organizing themselves. Social media and online platforms have increased the possibilities for self-organization through improved connectivity. In practice, we see that governments struggle to deal with this form of self-organization, while it also offers a unique opportunity to increase the response capacity. The smart use of citizens’ initiatives offers opportunities and can increase the effectiveness of the government’s action. This article focuses on the following question: what role do online platforms play in smartly guiding the self-organization of citizens during crises and disasters? We answer the question on the basis of two examples: the role of online platforms in the aftermath of the earthquakes in Nepal in 2015, and the coordination of the reception of refugees during the crisis in the Netherlands in the winter of 2015-2016.

    Lack of safety leading to death or injury has many aspects: personal violence, traffic, accidents at home or the work place, not to mention other external sources as radiation and fine dust or internal sources like diseases. In this paper we restrict ourselves to the lack of safety in traffic as a system, and address the following questions: (1) Is traffic a system with one dominant responsible actor (Ministry of Traffic)? (2) What explanations are offered for the lack of safety? (3) Who is responsible for safety? (4) What sort of interventions are suitable for enhancing safety? and (5) What type of process/consultancy is most appropriate for answer the preceding questions?
    The paper arrives at its conclusions via an indirect way, namely a short analysis of what a system comprises and what constitutes an explanation, responsibility and effectiveness. It appears that we are all causally involved in the lack of safety, and we are all responsible at all levels of society: micro-individual, meso-organizational and macro-societal. Therefore a promising route to maximize safety is a governance perspective comprising multiple stakeholder co-responsibility. It remains to be seen whether this perspective should replace the predominant command and control governance or complement it.

    The crowd increasingly plays a key role in facilitating innovations in a variety of sectors, spurred on by IT-developments and the concomitant increase in connectivity. Initiatives in this direction, captured under the umbrella-term ‘crowd-based innovations’, offer novel opportunities in socio-technical systems by increasing the access, reach and speed of services. At the same time, they signify important challenges because these innovations occur in a context of traditional, well-established institutional and governance structures and practices. This dynamic is captured in the idea of the ‘institutional void’: the tension between traditional structures and (radically) new initiatives. Existing rules, standards and practices are challenged, which raises questions about the safeguarding of public values such as quality, legitimacy, efficiency and governance of crowd-based innovations. This article argues that understanding these tensions requires supplementing empirical research with an explicitly normative dimension to reach thorough and balanced conclusions to facilitate innovation while protecting the valuable elements in existing rules and regulations. Illustrated by a number of short examples, we propose a multidisciplinary research agenda towards formulating appropriate governance structures.

    In The Netherlands an innovative water safety policy is under development: multi-layered safety. This innovation is a move from a preventive approach (levees) towards a risk approach. Mitigation of consequences for spatial measures and disaster management too are considered in reducing flood risks. The theory of path dependence teaches us that many technical, financial and institutional factors keep the current policy system in its equilibrium. This complicates policy innovations. This article contains a case study that explains how pilots contribute to a process of policy innovation. It concludes that enshrining results in the ‘home organizations’ and synchronizing pilots with running policy processes is essential. The pilots also show that policy renewal concerns a process of ‘muddling through intelligently’.

    In the safety domain important developments are currently taking place concerning the way in which administrative and tactical operations are dealt with in crises. It is characteristic for the approach that cooperation in chains and networks is increasingly needed to arrive at a suitable approach. In this essay, the authors analyse two subdomains in two concrete cases on how cooperation between several parties takes shape and functions. For that purpose, they differentiate between acute and non-acute crises. In the subdomain ‘acute crises’, they have chosen the case of the Dutch town Moerdijk (the fire at the Chemie-Pack company in 2011). In the subdomain of non-acute crises, they focus on community safety partnerships (‘Veiligheidshuizen’), especially on the community safety partnership in the Dutch province of Friesland (‘Veiligheidshuis Friesland’). In both subdomains the establishment of a good basic cooperation and leadership structure appears to be of prime importance. From there it is necessary to respond in a flexible manner (to be able to execute custom-made work). Within that framework, the capacity may develop to arrange a well-structured and effective cooperation at the operational level (ad hoc in acute crises), to monitor progress properly and to carry out targeted interventions if the developments in the situation ask for these interventions. A good knowledge of each other’s frame of reference is necessary to make this work, so that a maximum level of integration in the approach is achieved.

    Politicians and scientists in the Netherlands often claim that only municipalities with over 100,000 inhabitants (so called ‘100,000+ municipalities’) have enough administrative power to be able to carry out their tasks in the future well. This is also the case for the responsibilities that recently have handed over to the Dutch municipalities as part of the three decentralizations. Against the background of this debate, the authors of this essay argue that the experiences of the four European microstates – Andorra, Liechtenstein, Monaco and San Marino –may offer an interesting frame of reference where it concerns the delivery of public services. These four countries have all the responsibilities and tasks of a sovereign state, but at the same time three of the four countries have a population of fewer than 40,000 inhabitants. Also, the fourth country is smaller than a 100,000+ municipality. Despite the small size of these states, their public services are of an exceptionally high level. Therefore this essay tries to answer two questions: How is this possible? What can we learn from the experiences of these microstates about the debate on scale and administrative power in the Netherlands?

    The Netherlands enjoys a long-standing reputation in the field of road safety. Following the ‘Sustainable Road Safety’ program of the 1990s, the infrastructural layout considerably reduces the risk of accidents while at the same time many measures are in place to improve road user behavior. In recent years, however, there have been several developments that may affect the success of the approach. In addition to an ageing population, urbanization, and the rise of new technologies, there is a combination of decentralization, policy integration, and budgetary restraints.
    This contribution presents the philosophy behind the sustainable road safety program and describes its key elements. It discusses policy and societal developments, and the continuing need to improve road safety, especially at a decentralized level. The article argues in favor of a continued systematic approach towards safer road traffic, also in periods of decentralization and limited budgets. It presents a concrete perspective of the role of policy analysis and policy evaluation to develop a proactive approach to road safety under the new conditions. In this, evidence-based interventions, an active use of safety performance indicators, and a focus on success are key elements.

    In the spring of 2013, the Dutch railway operator, High Speed Alliance, cancelled the acceptance procedures for V250 high-speed train sets they ordered from Italian manufacturer AnsaldoBreda after numerous problems with the build quality and safety systems. It is the latest chapter in a series of failed attempts to build and run a high-speed railway link between Amsterdam and Paris. Inevitably, the question of who was responsible for the decision to order the V250 has become prominent. We study 20 years of public decision-making by analysing the content of over 1,200 documents. Using David’s and Arthur’s theory of path-dependency, we conclude that all the decisions that were taken during those two decades created a situation in which the choice for the V250 was nothing more than an inevitable gamble. It can therefore be concluded that all parties involved, from the railway operator to Parliament, have contributed to the current failure.

    In this contribution we analyze the content of the Veerman Commission’s report that in 2007/2008 advised the Dutch government on the necessity of measures to protect the coast against future rising waters and other climatological and environmental changes and challenges. We conclude that the content of the report is in itself an expression of the ongoing social and governmental debate and process that tries to create a sense of urgency since there is no real immediate crisis to facilitate changes. Thus the report is not only the result of the work of the commission (though its firm statements on the necessity of measures were clearly heard), but at the same time the reflection of an ongoing debate which also creates a new challenge for stakeholders in the water domain. The organizational recommendations of the commission to place the protection against rising waters as far away from day to day politics as possible have all been put into action, which is a noteworthy result. Still, it is the ongoing process between stakeholders that will determine the actual measures to be taken by the government water related bodies.

    Problems with the rivers were the incentive to change Dutch flood safety policy in 2000. ‘A different approach to water’ became the new policy slogan. Government presumes that public engagement in decision-making on flood safety will lead to more water awareness and risk aware behaviour. ‘No or hardly any negative side-effects’ are expected from this strategy. However, our research shows that public engagement in flood safety decision-making leads to fierce policy contestation, since proposed solutions do not meet the safety perception of local citizens. ‘A different approach to water’ strategy does not resonate in Dutch society because most people are convinced that government guarantees water safety. In this article we explain that this so-called ‘myth of water safety’ is based on a deep frame of risk control. We argue that flood safety needs to be reframed and that a new frame should be based on acceptance of vulnerability instead of risk control.

    By law Dutch companies providing drinking water have to remain in public hands. In practice, public shareholders often fulfil their role in an informal fashion and without much engagement. They hardly intervene to protect the public interest. Public ownership is deemed to be crucial but its effects are difficult to measure. In this article, we examine the (ever changing and varying) institutional conditions of two public water companies and discuss whether these conditions enable public shareholders to properly control these companies. By comparing the opinions and experiences of directly involved respondents, this article aims to answer the question whether and how public shareholders are able to act as regulators. There are indications that this kind of regulation will not be efficient enough when needed, though those involved do not see this as a problem. This article offers suggestions for municipalities and provinces to reflect more on their role in the drinking water sector.

    This article discusses the changes in the safety policy in the Netherlands over about the last fifteen years. These changes are analysed as reactions to the problems that the police and other criminal justice agencies face and which result from the shift from a modern to a late modern society. Five main changes are distinguished: in the organisational and managerial arrangements of the police; in the relation between the state (police) and other (both public and private) agencies; the rise of extra-judicial instruments and the growing attention for the position of victims; the increasing use of technological instruments for surveillance and crime prevention; and a harsher and more punitive policy. These changes create new fundamental questions for a future safety policy.

    Over the last decades, the number of European Union member states has significantly increased, resulting into a reduction of the relative formal power of the Dutch government. Improving Dutch influence in Brussels is therefore an important topic on the agenda of public administration scholars and advisory boards. Using experts at the various stages of the EU policymaking process is one option to increase the influence in the EU. This article evaluates the effectiveness of this strategy concerning one of the most complex and most controversial public policy issues in the history of the European Union: the revision of the EU's chemical policy (REACH). The article demonstrates that the 'expert strategy' has been successful in this case. However, the effectiveness of this strategy comes under pressure if the trend towards core departments will continue.

    Uncertainties about climate change are a major incentive for the Dutch government to communicate frequently about risks related to water safety. In September 2008, the 'New Delta Committee' even presented several coping strategies to safeguard the low-lying Delta of The Netherlands far into the next century. The government assumes that increased high water risk awareness and behaviour in society, based on a risk-based approach, is an important factor for sustainable future living with water and thus made this into the spearhead of policy. However, the main part of Dutch society still lives in a flush of victory. The Delta Works symbolize the victory over the water; The Netherlands is safe. The government doesn't succeed satisfactorily to realize high water risk awareness and behaviour in its society, notwithstanding years of campaigning. Cultural sociologists Heems and Kothuis demonstrate by means of a discourse analysis of public campaigns that government communication on high water safety is not only entangled but also creates confusion. Reason of the entanglement in communication is a breakthrough of the taboo on publicly speaking of a flood disaster as a realistic scenario. The confusion obstructs the Dutch government to bridge the gap in perception between itself and society and to achieve its policy objectives.
